Biography: Daniel Elshoff, MD joined Premier Physician Network/Upper Valley Medical Center in 1999. A graduate of Wright State University School of Medicine with a specialty in Family Medicine, Dr. Elshoff completed his residency at Miami Valley Hospital. He is a member of the American Academy of Family Practice and the Ohio State Medical Association and has his Ohio Medical license.

Biography: Kathleen N. Kelly, PA-C received her Master of Science in Physician Assistant Studies in 2016 and her Bachelor of Science in Human Biology in 2014, both from Kettering College. Biography: Pamela M. Madden, CNM earned a midwifery certificate in 1999 from the Institute of Midwifery, Women and Health in Philadelphia. The following year, she earned her master's degree in midwifery from the Philadelphia College of Textiles and Science.

Biography: Julie Larson, MD is a graduate of Carroll College and the University of Washington School of Medicine. She completed her residency in family medicine at the University of Utah at Salt Lake City. Dr. Larson is certified in family practice and geriatrics by the American Board of Family Physicians. She has a special interest in women's health and geriatrics. Dr. Larson has been with Premier Health Family Care -North since 2002.

Prior to joining the Center for Women's Health and Wellness, she served as a registered nurse in labor and delivery at Atrium Medical Center. Biography: Dr. Hester graduated from Wright State University Boonshoft School of Medicine. He completed his residency in internal medicine at Miami Valley Hospital and Good Samaritan Hospital (closed in 2018). Dr. Hester is certified by the American Board of Internal Medicine.

Biography: Dr. Agarwal is a graduate of Jiwaji University in Madhya Pradesh, India. She completed residencies in pathology and internalmedicine at Miami Valley Hospital and Wright State University. Dr. Agarwal is board certified in internalmedicine. She has more than 20 years of experience as a family practitioner.

Dr. Kohnen is certified in Family Practice by the American Board of Family Physicians. Dr. Kohnen has been with Beavercreek Family Physicians since 1997.
