Biography: Elizabeth R. Swartzwelder-Cozad, MD graduated from Wright State University Boonshoft School of Medicine. She completed her family medicine residency at Wright State University. Dr. Swartzwelder-Cozad is a member of the American Academy of Family Physicians, Ohio Academy of Family Physicians, and the American Medical Association. She is board certified in family medicine.

Biography: Terez Metry, MD is a graduate of Zagazig University in Egypt. She completed her Internship in Internal Medicine at St. Mary's in Rochester, New York and her residency at Miami Valley Hospital in Dayton, Ohio. Dr. Metry is certified in Internal Medicine by the American Board of Internal Medicine. She has special interest in Women's Health and Diabetes. Dr. Metry has been associated with Premier Physician Network since 1996.

Biography: Jeffrey Rayborn, MD is a graduate of The Ohio State University and the University of Cincinnati College of Medicine. He completed his residency in Family Practice and his fellowship in Sports Medicine at Bethesda North Hospital. Dr. Rayborn joined Premier Orthopedics in 2009. Dr. Rayborn's practice focuses on non-operative orthopaedic care for adolescents and adults, and primary care sports medicine. Dr. Rayborn also employs the use of musculoskeletal ultrasound in the evaluation of orthopaedic conditions, as well as in the placement of needles to support the aspiration or injection of joints such as the shoulder, elbow, hip, and knee. The use of musculoskeletal ultrasound has been shown to enhance patient management and outcomes in both general orthopaedic and sports medicine populations. Dr. Rayborn also offers Platelet Rich Plasma Injections (PRP) when medically appropriate. Dr. Rayborn is actively engaged in the sports medicine community. He currently serves as the Medical Director for Upper Valley Sports Medicine Center and is also the team physician at Tipp City High School. Dr. Rayborn is a member of the American Medical Society for Sports Medicine and the American Institute of Ultrasound in Medicine. He maintains office hours at Miami Valley Hospital North in Englewood and the Hyatt Center in Tipp City. Dr. Rayborn is a Credentialed ImPACT® Consultant trained in treating concussions as well as administering and interpreting the ImPACT® test.

Biography: James J. Klosterman, MD joined Premier Orthopedics in 1997 after fulfilling his service commitment in the United States Air Force. He is a graduate of the University of Dayton and the Cincinnati College of Medicine, and completed his residency in Orthopaedic Medicine at the Medical College of Virginia. Dr. Klosterman provides operative care for a broad set of orthopaedic conditions, including arthroscopy of the knee and shoulder and joint replacement of the hip, shoulder and knee. He is credentialed to offer the Uniknee™ replacement option to suitable candidates. Dr. Klosterman is a member of the UD Sports Medicine Staff. He is well respected in the Miami Valley and acts as the orthopaedic consultant to area high school and club athletic programs. In 2007, Dr. Klosterman was selected as the Collegiate Team Physician of the Year by the Ohio Athletic Trainers Association. He is board certified by the American Board of Orthopaedic Surgeons. Dr. Klosterman holds an appointment as an Assistant Clinical Professor, Wright State University School of Medicine. Biography: Joseph S. Leithold, MD has been in practice in Beavercreek since 1996. After spending the beginning of his career as a physician's assistant, Dr. Leithold decided to attend Wright State University's School of Medicine to become a physician. Dr. Leithold - who prefers his patients call him Dr. Joe - completed his internship and residency at Geisinger Medical Center in Pennsylvania, then returned to provide family practice care to the Dayton area. Certified by the American Board of Family Practice, Dr. Leithold is committed to providing quality care to you and your family. Dr. Leithold lives in the Dayton area with his wife and their two sons. Biography: Sanford M. Wolfe, DO, graduated from the College of Osteopathic Medicine. He completed residency in internal medicine at the U.S. Air Force Medical Center at Wright Patterson Air Force Base. Dr. Wolfe completed his fellowship in rheumatology at the University of Tennessee Center for the Health Sciences in Memphis, TN. Dr. Wolfe has practiced rheumatology since 1986. Dr. Wolfe is board certified in internal medicine and rheumatology.

Biography: Joseph P. Allen, MD, FAAFP is board-certified in family medicine and is a graduate of Wright State University School of Medicine. He completed his residency through the Dayton Community Family Practice Residency Program at Good Samaritan Hospital (closed in 2018). Dr. Allen's special interests include pediatric health, sports medicine and preventive medicine. Dr. Allen is also the Premier Physician Network Regional Medical Director. He lives in Dayton with his wife and two children. Dr. Allen is a Credentialed ImPACT® Consultant trained in treating concussions as well as administering and interpreting the ImPACT® test.

Biography: Anoopa Hodges, DO has shared her expertise in managing acute and chronic illness in the adult population, including geriatrics, since 2004. Dr. Hodges has focused on preventative care medicine in a personal and friendly atmosphere since 2009. As a doctor of osteopathic medicine, she is able to perform manipulative treatment as well. As a Primary Care Physician, she oversees all aspects of a patient's care and refers to specialists when necessary. She is Board Certified by the American Board of Internal Medicine.

Biography: Ronald Pohlman, MD is a graduate of the University of Dayton and The Ohio State University College of Medicine. He completed his residency in Family Practice at St. Elizabeth Medical Center in Dayton. He is certified in Family Practice by the American Board of Family Physicians, and in Hospice and Palliative Medicine by the Academy of Hospice and Palliative Medicine. Dr. Pohlman has been with Beavercreek Family Medicine since 2000.
