Biography: Jeffrey Rayborn, MD is a graduate of The Ohio State University and the University of Cincinnati College of Medicine. He completed his residency in Family Practice and his fellowship in Sports Medicine at Bethesda North Hospital. Dr. Rayborn joined Premier Orthopedics in 2009. Dr. Rayborn's practice focuses on non-operative orthopaedic care for adolescents and adults, and primary care sports medicine. Dr. Rayborn also employs the use of musculoskeletal ultrasound in the evaluation of orthopaedic conditions, as well as in the placement of needles to support the aspiration or injection of joints such as the shoulder, elbow, hip, and knee. The use of musculoskeletal ultrasound has been shown to enhance patient management and outcomes in both general orthopaedic and sports medicine populations. Dr. Rayborn also offers Platelet Rich Plasma Injections (PRP) when medically appropriate. Dr. Rayborn is actively engaged in the sports medicine community. He currently serves as the Medical Director for Upper Valley Sports Medicine Center and is also the team physician at Tipp City High School. Dr. Rayborn is a member of the American Medical Society for Sports Medicine and the American Institute of Ultrasound in Medicine. He maintains office hours at Miami Valley Hospital North in Englewood and the Hyatt Center in Tipp City. Dr. Rayborn is a Credentialed ImPACT® Consultant trained in treating concussions as well as administering and interpreting the ImPACT® test.

Biography: Michael G. Raab, MD received his undergraduate degree in Chemical Engineering at the University of Notre Dame (1985), and his medical degree from the University of Illinois, Chicago (1989). He completed his residency training in orthopedic surgery at Southern Illinois University School of Medicine, Springfield, in 1994, and received the Outstanding Surgical Resident Teaching award. Dr. Raab then served as an orthopedic surgeon at Eisenhower Army Medical Center, Ft. Gordon, GA, providing a wide range of general orthopedic surgery services, including trauma and fracture care;sports injuries;total joint replacement; shoulder, hand and elbow surgery; and hip, knee, and ankle surgery. He was a member of the Joint Replacement Service, andChief of the Handand Upper ExtremitySurgery Service. He was also a member of the teaching faculty for the Army orthopedic residency training program. Following four years of active duty, Dr. Raab practiced in Georgia and Ohio, and joined Premier Orthopedics in 2011. He is Board Certified in orthopedic surgery and a fellow of the American Academy of Orthopedic Surgeonsand American College of Surgeons. He is a member of the Society of Military Orthopedic Surgeons andthe American Association for Hand Surgery. Biography: Jennifer Jerele, MD, received her medical degree at Loma Linda University School of Medicine, and completed her residency at Wright State University Boonshoft School of Medicine. Dr. Jerele completed her fellowship at Duke University Medical Center (NC). Dr. Jerele is board certified in orthopedic surgery.

Biography: Shelden C. Wical, DO is a family medicine physician who earned his Doctor of Osteopathic Medicine from Lake Erie College Of Osteopathic Medicine in Erie, PA. He completed his family practice residency and internship at OUCOM/Doctors Hospital in Columbus, OH.Dr. Wical has been the team physician for Fort Loramie High School athletics since 2009. He is a member of a number of professional societies including: American Osteopathic Association, American College of Osteopathic Family Physicians, Catholic Medical Association, and Tennessee Osteopathic Medical Association. Dr. Wical is board certified by the American Osteopathic Board of Family Practice in family medicine and osteopathic manipulative treatment.

Biography: James J. Klosterman, MD joined Premier Orthopedics in 1997 after fulfilling his service commitment in the United States Air Force. He is a graduate of the University of Dayton and the Cincinnati College of Medicine, and completed his residency in Orthopaedic Medicine at the Medical College of Virginia. Dr. Klosterman provides operative care for a broad set of orthopaedic conditions, including arthroscopy of the knee and shoulder and joint replacement of the hip, shoulder and knee. He is credentialed to offer the Uniknee™ replacement option to suitable candidates. Dr. Klosterman is a member of the UD Sports Medicine Staff. He is well respected in the Miami Valley and acts as the orthopaedic consultant to area high school and club athletic programs. In 2007, Dr. Klosterman was selected as the Collegiate Team Physician of the Year by the Ohio Athletic Trainers Association. He is board certified by the American Board of Orthopaedic Surgeons. Dr. Klosterman holds an appointment as an Assistant Clinical Professor, Wright State University School of Medicine. He is a fellow of the American Academy of Orthopaedic Surgery, and a member of the American College of Sports Medicine and the Ohio State Medical Association.

Biography: Kathryn Lorenz, MD is a board certified family practice physician who earned her medical degree from the Boonshoft School of Medicine, Wright State University, Dayton, OH. She completed a family practice residency at Dayton Community Family Medicine, Dayton, OH. Dr. Lorenz joined Premier Physician Network in 2009. Dr. Lorenz received many honors during her training: Introduction to Clinical Medicine Award, Montgomery County Medical Society Alliance Scholarship, and the Ranville Scholarship. She was also a finalist for Outstanding First Year Medical Student.

Biography: Tammy J. Taylor, DO is board certified in pediatric medicine. Dr. Taylor received her medical degree from the Ohio University College of Osteopathic Medicine, Athens, OH, followed by her residency at Doctors Hospital, Columbus, OH. During her residency, Dr. Taylor held the position of Chief Resident in Pediatrics. She was also honored with the Community Service award for her work with the Pediatric Clinic. Dr. Taylor received other numerous honors and awards during her medical school and undergraduate education. She joined the Premier Physician Network, The Pediatric Group in 2003.
