Biography: Jay Hoffman, MD is a graduate of Duke University and the Wright State University School of Medicine. He completed his residency in internal medicine at Kettering Medical Center in Dayton. Dr. Hoffman is certified by the American Board of Internal Medicine. He has been with SureCare Medical Center since 1995.

Care Philosophy: I take pride in providing my patients with individualized, patient-centered care. I believe every woman has unique needs and goals, and our care team works to provide a full range of OBGYN healthcare to serve those varied needs. Pregnancy and birth are very special events in the lives of women. I believe that this experience is unique for each woman. I see my role as supportive of the woman and her family, understanding that ultimately the experience of pregnancy and birth belongs to them. Our team serves families as guides during this natural process. We feel privileged to participate in this journey. I am also happy to assist patients with menstrual issues, fertility, family planning, hormonal concerns, pelvic pain, infections, as well as general health maintenance screenings. I focus on patient education whenever possible. Biography: Richard Surowiec, MD received his medical degree from Saint Louis University in 2006. Dr. Surowiec completed a combined internal medicine and pediatrics residency from Wright State University in 2010. Dr. Surowiec is board certified in internal medicine.

Care Philosophy: Dr. Schulz is a family medicine physician who emphasizes high quality, patient-centered care. He treats patients of all ages, from newborns to seniors, and enjoys caring for multi-generational families. With a focus on compassionate care, Dr. Schulz treats a wide range of chronic conditions, including diabetes, heart disease, carpal tunnel syndrome, arthritis, high blood pressure, and more. Biography: Julia A. Boyd, MD is a graduate of the University of Dayton and The Ohio State School of Medicine. She completed her residency in family practice at Good Samaritan Hospital (closed in 2018) in Dayton. Dr. Boyd is certified in family practice by the American Board of Family Physicians. She has been with South Town Family Practice and Premier Physician Network since October 2006.

Biography: Anessa Alappatt, MD is a graduate of Northeastern Ohio Universities College of Medicine. She completed her residency in family medicine at Indiana University's Methodist Hospital. Dr. Alappatt is board certified by the American Board of Family Medicine. She has been with Fairborn Medical Center and Premier Physician Network since January 2008.

Biography: Paul Jennewine, MD has been in practice in Middletown since 1997 and is board certified in internal medicine. He joined Middletown Medical Group after finishing his residency and receiving his medical degree at the University of Toledo Medical Center. A member of the American College of Physician and the Ohio State Medical Society, Dr. Jennewine also has served two terms as president of the Bulter County Medical Society and is the medical director of the Middletown Health Department. Dr. Jennewine currently lives in Middletown with his wife and three daughters.

Biography: Julia E. Paulet, MD is a graduate of West Virginia University School of Medicine in Morgantown, WV. She completed her OB/GYN residency at Wright State University Boonshoft School of Medicine in Dayton, OH. She is a native Daytonian who grew up in Vandalia. She is looking forward to taking care of women in the area and is passionate about providing health care to all generations. In her free time she enjoys spending time with her husband and dog.

Biography: Elizabeth R. Swartzwelder-Cozad, MD graduated from Wright State University Boonshoft School of Medicine. She completed her family medicine residency at Wright State University. Dr. Swartzwelder-Cozad is a member of the American Academy of Family Physicians, Ohio Academy of Family Physicians, and the American Medical Association. She is board certified in family medicine.

Biography: Terez Metry, MD is a graduate of Zagazig University in Egypt. She completed her Internship in Internal Medicine at St. Mary's in Rochester, New York and her residency at Miami Valley Hospital in Dayton, Ohio. Dr. Metry is certified in Internal Medicine by the American Board of Internal Medicine. She has special interest in Women's Health and Diabetes. Dr. Metry has been associated with Premier Physician Network since 1996.

Biography: Pam Werner, MD is a graduate of Wright State University and The Ohio State University College of Medicine. She completed her residency in Internal Medicine at Miami Valley Hospital in Dayton. Dr. Werner is certified in Internal Medicine by the American Board of Internal Medicine. She has been with Premier Health Primary Care - Brown Street, previously known as Miami Valley Primary Care, since February 2003.

Biography: Julie Myers, DO is a graduate of the University of Dayton and Ohio University College of Osteopathic Medicine. She completed her residency in the Wright State University Internal Medicine and Pediatrics Program. Dr. Myers is certified in Pediatrics and Internal Medicine. She has been with the Internal Medicine and Pediatrics of Centerville since 1993.

Biography: Brent Imbody, MD graduated from Wright State University, School of Medicine in Dayton, Ohio and performed his residency at Michigan State University. He holds current memberships with the American College of Obstetrics and Gynecology, the Ohio State Medical Association and Montgomery County Medical Society. Dr. Imbody is an Associate Clinical Instructor and a lecturer at Wright State University. He formerly served as Chairman of the Miami Valley Hospital Ob/Gyn Department. Dr. Imbody has been in private practice since 1992. He has specialized training in endometrial ablations, and is also experienced in minimally invasive hysteroscopic sterilization, intrauterine devices and minimally invasive hysterectomies. Dr. Imbody also specializes in hormone replacement therapy as well as bio-identical hormones.
