Biography: Jeffrey Rayborn, MD is a graduate of The Ohio State University and the University of Cincinnati College of Medicine. He completed his residency in Family Practice and his fellowship in Sports Medicine at Bethesda North Hospital. Dr. Rayborn joined Premier Orthopedics in 2009. Dr. Rayborn's practice focuses on non-operative orthopaedic care for adolescents and adults, and primary care sports medicine. Dr. Rayborn also employs the use of musculoskeletal ultrasound in the evaluation of orthopaedic conditions, as well as in the placement of needles to support the aspiration or injection of joints such as the shoulder, elbow, hip, and knee. The use of musculoskeletal ultrasound has been shown to enhance patient management and outcomes in both general orthopaedic and sports medicine populations. Dr. Rayborn also offers Platelet Rich Plasma Injections (PRP) when medically appropriate. Dr. Rayborn is actively engaged in the sports medicine community. He currently serves as the Medical Director for Upper Valley Sports Medicine Center and is also the team physician at Tipp City High School. Dr. Rayborn is a member of the American Medical Society for Sports Medicine and the American Institute of Ultrasound in Medicine. He maintains office hours at Miami Valley Hospital North in Englewood and the Hyatt Center in Tipp City. Dr. Rayborn is a Credentialed ImPACT® Consultant trained in treating concussions as well as administering and interpreting the ImPACT® test.

Biography: Susan F. Davis-Brown, MD is a graduate of Wright State University and the University of Cincinnati College of Medicine. Dr. Davis-Brown's postgraduate training was at the University of Michigan Hospitals/St. Joseph Mercy Hospital in Ann Arbor. Dr. Davis-Brown is board certified in Internal Medicine and Pediatrics. She has a special interest in preventative medicine, risk factor reduction, women's health, and diabetes. Dr. Davis-Brown joined Brookville Family Care in 2006.

Biography: Polina Sadikov MD, is a graduate of 1st Medical School in the USSR. She completed her residency in family medicine at Wright State University in Dayton, OH. Dr. Sadikov is board certified in family practice by the American Board of Family Medicine. She is a member of the American Medical Association and the American Academy of Family Physicians. Dr. Sadikov is a dedicated family medicine physician with a passion for comprehensive care. She provides personalized, patient-centered treatment for a wide range of conditions, including diabetes, hypertension, heart disease, musculoskeletal disorders, depression, anxiety, thyroid disorders, obesity, asthma, COPD, and menopause. Biography: Leelmohan Ravikumar, MD graduated from Ross University School of Medicine, North Brunswick, New Jersey in 2009. Dr. Ravikumar completed his residency in family medicine at Clinton Memorial Hospital/University of Cincinnati College of Medicine in 2013. Dr. Ravikumar is board certified in family medicine. Dr. Ravikumar is accepting new patients, including pediatrics.

Biography: James Dunlap, MD, MBA, is a graduate of Georgetown University School of Medicine in Washington, DC. He completed his residency in Orthopedic Surgery at University of Kansas School of Medicine - Wichita in Wichita, KS. He went on to complete a fellowship in Spine Surgery at the University of Pittsburgh Medical Center in Pittsburgh, PA. Dr. Dunlap is board certified in Orthopaedic Surgery by the American Board of Orthopaedic Surgery. Dr. Dunlap is dedicated to providing compassionate, patient-centered care for neck and back pain. He specializes in non-operative spine evaluation and treatment, emphasizing personalized, evidence-based approaches to restore mobility and improve quality of life. Dr. Dunlap believes in empowering patients through education and shared decision-making, ensuring they understand their condition and actively participate in their recovery. He collaborates closely with a multidisciplinary team, including physical therapists and pain specialists, to provide comprehensive care tailored to each individual's needs. He completed his residency through the Dayton Community Family Practice Residency Program at Good Samaritan Hospital (closed in 2018). Dr. Allen's special interests include pediatric health, sports medicine and preventive medicine. Dr. Allen is also the Premier Physician Network Regional Medical Director. He lives in Dayton with his wife and two children. Dr. Allen is a Credentialed ImPACT® Consultant trained in treating concussions as well as administering and interpreting the ImPACT® test.

Biography: Christopher Lauricella, DO is a graduate of Florida State University and Ohio University College of Osteopathic Medicine. He completed his residency in Family Practice at Grandview Medical Center in Dayton. Dr. Lauricella is certified in Family Practice by the American College of Osteopathic Family Physicians. Dr. Lauricella has a special interest in cardiology and sports medicine. He has been with Premier Physician Network since November 2006.
