No recollection
I have no recollection seeing this provider while in the hospital yet I have her card in my pocketbook and a rx by her. She prescribed me a medicine my mother, bf, brother, and I all unformed her I could not take yet wrote in my chart I 'refused' to take said rx. I have been on a different medication for a decade. I will be filing a complaint with the DA and I was treated very disrespectful and not slowly taken off of previous medication.
