I attended one of your events and was mislead by your office worker upon signing up for your $49 promotional assessment and treatment plan,i inquired about which insurance you accept i was told my insurance is not accepted, your receptionist said she will make the appointment and i just pay the $49 out of pocket that most insurance will reimburse you,I told her that my insurance does not cover out of network i would just do the assessment and treatment plan but i will not do followup treatment she was fine with that and booked me for the assessment and treatment plan, upon arriving the next day i was given 10 forms to be read and signed due to vision problems i informed the receptionist and you i am not comfortable with signing the forms i could not read,i would take the forms home, when my daughter was assisting me with the forms she questioned why do they need your social security number and all this personal information if you are paying out of pocket,and you're not following up with their office, during a phone call your receptionist was very rude and would not further answer the question. what does the assestment entail? She kept repeating a assessment with a treatment plan, and by law we have to have your social security number to be seen, at this point i asked for a refund and at end of the conversation she stated she would see what she could do to get my refund,I never heard from your office,i sent you 2 emails and you never responded, i am very much disturb the fact that you as a professional person would not respond to a $49 request for refund due to the fact that you did not rendered any service. I would prefer to resolve this issue and restore peace in my life without feeling that you took my hard earn money. thanks!
