Did not address my main concern for visit to urgent care. She gave me another patient’s discharge paper instead of my own. Gave me someone else’s doctors release from work paperwork and booted me out the door before my imaging test came back to determine what was going on with my health. She also falsified vitals and observations upon intake. Included in my visit summary were someone else’s diagnosis that had nothing to due with the parts of my body I was there for! I think she has been in practice for many years past her prime. And HIPPA violations by giving me someone else’s paperwork with all of their personal and medical info on it just top it off! Time for a retirement home and some meds for her senile tendencies!
