My name is Carolyn Tellez, and I am a board certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). I am licensed in Arizona, Nevada, and Colorado. My focus is on medication management, but I always encourage incorporating lifestyle, and behavioral changes for the best outcome possible.
