Cannot perform a neurological assessment
My father was discharged from her care at St. Joseph's after suffering a stroke. He is now permanently disabled. Dr. DeSouza's discharge summary is the most incomplete document I've ever read. She only bothered to document 'alert' for his neurological exam. There is ZERO mention of his multiple new neurologic deficits including severely diminished motor function on half his body, inability to walk, and altered mentation...... In medical school we are taught much better! It is heartbreaking to see that such a thoughtless and incompetent physician was in charge of my father's care, and I worry for all of the patients who fall under the care of such a physician.
