I was sent an invoice by this office for medical services performed. I have never been to Kansas and have never visited Dr. Mitchell. I tried to clear up via phone but was unsuccessful. When I called the office, a person who was not very good at English asked me for a copy of my drivers license and social security via fax. I was appalled.
