I requested a copy of a doctor's note Dr. Estrada had issued to me a few years ago. It was part of an appeal that required a medical history. In lieu of a copy of the note, Dr. Estrada could have written a letter. Dr. Estrada refused to write the letter because she said it was strange that we needed my medical history now. The note was in storage and took weeks to get. This prevented me from filing the appeal in a timely manner and cost me hundreds of dollars.
