LIES ON HIS CHARTS. Thank goodness for having access to my medical visit notes. He auto-populates charts with information from 2013 and doesn't ask. No vitals, no review of systems. Was in the room for 5 minutes. Looked at my skin. Prescribed something. Didn't advise on adverse events to look out for, didn't explain the causes or prevention for my condition. Seemed like he was in a hurry. On the chart he stated that I declined an exam which he didn't offer. This was a very problem focused, level 1 visit, but he decided to charge my insurance a level 3 visit, despite the lack of details in history taking and physical exam.
