Dr. Kozak was brusque to the point of being rude, and disregarded just about anything that I had to say. She misdiagnosed me twice in visits a couple of days apart and I ended up in ER being treated for something she should have easily seen. She is not open to questions and was dismissive of my concerns. I do not recommend her at all.

Did not follow thru with my mother and her abdominal pain. She was dead within 5 weeks. Acknowledge pain and said "that's not good" and sent her home. Would give no stars if possible

Dr.Kozak has been my physician for the past 20 years. She has always been very professional, very helpful and extremely knowledgable. I have referred several members of my family and they are very satisfied with her. I have had several very serious physical situations and she was on top of them immediately. We should be lucky enough to have more health care members like Dr. Kozak.

Condescending and uncaring. These two sentiments comes through clearly during my first visit with her. I have always seen the PA and often wondered why the staff in general on my first and subsequent visits appeared so abrupt and disinterested in the patient. Now that I met the doctor, I can see why. An organization is a reflection of those who leads it. When you are truly ill, you need to at LEAST feel like those who are treating you care, even if they don't. This is not the doctor you want to
