I went for service in November 2024 for my son. A vision exam is FULLY covered on my insurance with zero copay.Because I saw other complaints on Google that this provider might submit unauthorized charges, I even made written notes on all of my intake forms saying that I only consented to what was covered fully under the vision plan. Before I left, I confirmed that my balance was zero.However, I just received a bill in the mail for 68 dollars for something called Fit & Follow-Up Premium. I did NOT purchase glasses from this provider because their prices were outrageous, so how exactly do they feel entitled to charge a Fit & Follow-Up Premium. NO glasses were fitted. Nor did we do a contact fitting. There was NO follow-up either.They also would not provide the PD (pupillary distance) either, so I paid out of pocket at another provider to obtain this.
