Doctor did take responsibility for patients me

I would like to say the visit was great but it was more if you have never had this test it might not be this. Had the test just to prove that where I moved from in Atlanta they were correct. Furthermore, twice they changed my medical history and it was a major disease. I’m sorry I don’t care if it was the person transcribing his notes, the Dr. is ultimately responsible for the accuracy of the information. Furthermore, I was unhappy with the Doctor and let the group know. Not one place in SGMC does it state if you mark this you are separating from the group. It is unclear and biased. It protects the Doctor but no one at any SGMC office or the hospital helps the patient or their medical records.
