I've had ongoing issues and recent ER visits and when contacting the office for assistance and follow-up Dr. Melendez nor his nurse Stacey has ever responded. Also, after reviewing my patient portal, office visits and notes are either missing altogether or are incorrect. If I didn't have concerns before I sure do now. This is unacceptable.
