Lost crown from implant
I had an implant with crown installed. After the implant was replaced once the new crown came loose in less than a year. When I called the office to get the crown re-installed they wouldn't see me. I asked for a referral and they wouldn't give me one. They told me that I would have to find another dentist to refit the crown. I did, and it cost over $2000. 92 Dentistry should have given me a referral and paid for the work.

Lost Crowns That Had to be Replaced
I had the same experience. I had years of great treatment, but then in July of the last year, Dr. Horacek left his practice for "personal reasons" and they kept saying he was coming back, but he never did. Now he has sold or left his practice without explanation or a referral. And now just a few days ago, I had a second crown that came out, this is the second such crown in the past six months. These crowns were supposed to be permanent and he got paid a lot from my insurance for this work when it was done. The two crowns were next to each other, I am left with only half a smile, and must see a new dentist, and so far, it looks like they can't replace one of them. I am also aware that Dr. Horacek in the last year was disciplined by the Board and fined for doing some implant work beyond the scope of his training, or without enough supervision when he was learning to do them. I do think he did good work initially, but his departure and recent work, has left quite a few of us in trouble.

Not at this time
My family has been using Dr Horacek for years and have been very happy. This year seems clinic has changed. This year they cancelled appointments, no call back to reschedule. My son had a dental emergency called a few times, no call back. We had to go to another dentist for care. Very sad, we are now looking for a new dental provider.
