Ratings mostly for the dental assistant, the dentist didn't need to spend a lot of time this visit so most of the survey questions don't relate to him. If you wanted to just rate him, there should be a N/A option for the questions also.

We brought our 4year old to Dr. Sebastian for a second opinion. He is thorough & patient explaining all treatment options with us. Dr. was awesome with our son! Highly recommend Dr. Sebastian & his staff!!
